Please call our office in advance at (203) 773-8959. The appointment must be scheduled by the patient or responsible party. You will need a Referral Form from your doctor. You may need Pre-Authorization from your insurance company, so please verify before your visit.

For your convenience, we have the following office hours:

Monday-Friday, 8:00 a.m. to 4:00 p.m.

What to Bring:

When coming to our office, please remember to bring a valid form of identification, your insurance card, and your referral form.

We ask all patients and their family members to please refrain from wearing any perfume, cologne, aftershave, or heavily scented lotions or hair products.
