Schedule An Appointment

Please call our office in advance at (203) 773-8959. Your appointment can be scheduled either by your doctor's office or by yourself. In either case, you will need a Referral Form from your doctor. You may need Pre-Authorization from your insurance company.

For your convenience, we have the following office hours:

Monday-Friday, 8:00 a.m. to 4:00 p.m.

What to Bring:

When coming to our office, please remember to bring along a valid form of identification, your insurance card, and your referral form.

We ask all patients and their family members to please refrain from wearing any perfume, cologne, after shave or heavily scented lotions or hair products. Dr. Hammers is highly allergic to fragrances.
