Please be sure to bring your insurance card and photo ID to your appointment. It is important for us to scan the image of your insurance cards. Specific questions regarding your policy and coverage should be directed to your insurance carrier, or your employer who provides any insurance to you, as insurance plans and benefits vary widely.
Insurance carriers who require preauthorization for radiological procedures require that the participating physician who referred you to our facility provide the insurance company with some clinical information concerning your medical condition and sometimes prior treatments.
It is the patient responsibility to get preauthorization. Please work with your referring physician and insurance company to ensure your preauthorization, when required, is received by Hammers and Riccio Imaging before your scheduled exam.
Without your insurance carrier's preauthorization number you may become fully financially responsible for any services rendered or your appointment time may need to be rescheduled.
